Registration
All new patients joining the practice will be asked to complete a registration form which includes a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Registration forms are available from the Practice. Patients can register using the online feature, link at the bottom of the page, however the practice will need our supplementary form to be completed also, this is in support of clinical care and safeguarding processes. Should a patient request proxy access for children under 16 years, picture ID will be required before access can be given.
Before registering, please check you are within our practice boundary. If you are not within our boundary, your registration will be rejected.
New patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Medical treatment is available from the date of registration. Please contact reception for further information.
