Welcome to Clayton Brook Surgery, providing health care for Clayton Brook and surrounding areas. The Surgery is situated in the Village Centre at Clayton Brook.

Clayton Brook Surgery joins Leyland Primary Care Network
Clayton Brook Surgery has recently joined Leyland Primary Care Network (PCN Home) (gpweb.org.uk) Leyland Primary Care Network is an NHS Collaboration between 6 GP Practices - Central Park Surgery, Clayton Brook Surgery, Moss Side Medical Centre, Sandy Lane Surgery, The Village Surgeries Croston & Eccleston, and Worden Medical Centre. The PCN staff include Clinical Pharmacists, Social Prescribers, Care Coordinators, Health and Wellbeing Coaches, First contact Physio, Paramedics and Care Home Leads. Our patient services will be enhanced by pharmacists carrying out medical audits and structured medicine reviews at Care Homes. Chronic Disease Management for housebound. Physio input into the MSK patient pathway. Paramedics will use enhanced clinical assessment and treatment skills to act as a first point of contact for patients with undiagnosed problems relating to minor illness or injury, such as chest pains and headaches. The Paramedic can triage patients, undertake telephone and face-to-face appointments, and carry out home visits (including residential and nursing homes) Social prescribers will assist with referral management to non-medical services such as the voluntary and community sector locally.

Friday 8:00 - 18:30
Saturday 8.30 - 12.30 - 3rd Saturday of the month
Sunday Closed
Please note the Surgery opens at 8.00 am but isn't fully operational until 8.30 am.
